You might need to screw tighten the plate or try installing the plate in the reverse direction. The area contacting the dovetail (the small silver wedge) might have gotten too smooth and the friction is not enough. I had the same issue when I accidentally crashed my rig (installing the pin on the first arm loosely which caused the the base of the arm to slip and bend under the weight) For now I have to tighten it very hard to get a secure grip
